If you determine that you do not wish to use the services, you may cancel at any time online at https://mypremiumbenefits.com/cancel, by calling our Customer Care Department at (888) 614-7973 or by notifying us in writing at Customer Service, My Premium Benefits 2655 Ulmerton Rd, Clearwater, FL33762. You will remain responsible for any unpaid membership fees, purchases, orcharges incurred prior to the effective date of cancellation. If the Company determines that it is unable to bill the membership fee to your designated billing source, the Company reserves the right to terminate this Agreement, in which event your access to and use of the services willimmediately cease. No refunds will be issued for fees or charges incurred prior to cancellation,except as otherwise required by applicable law or expressly provided under the terms of the program.
